ODI HAS RECEIVED 16 COMPLAINTS ALLEGING FAILURES OF THE IGNITION PARK-SHIFT INTERLOCK THAT ALLOWED THE IGNITION KEY TO BE REMOVED FROM THE IGNITION SWITCH WITHOUT THE TRANSMISSION BEING SHIFTED TO PARK IN MODEL YEAR (MY) 2002 AND 2003 HONDA ACCORD VEHICLES WITH AUTOMATIC TRANSMISSIONS. ELEVEN OF THE COMPLAINTS ALLEGE THAT THE FAILURE OF THE INTERLOCK RESULTED IN A ROLLAWAY CRASH. ONE OWNER ALLEGED THAT SHE WAS ALMOST INTO HER HOUSE WHEN SHE NOTICED THE CAR ROLLING OUT OF THE GARAGE AND DOWN THE DRIVEWAY AND THAT WHEN SHE TRIED TO STOP THE CAR IT ALMOST RAN OVER HER, BEFORE CRASHING INTO A CABLE BOX AT THE END OF THE DRIVEWAY. ANOTHER OWNER ALLEGED THAT THE VEHICLE ROLLED OUT OF THE DRIVEWAY AND STRUCK A TREE ACROSS THE STREET. THERE HAVE BEEN 28 COMPLAINTS MADE TO HONDA REGARDING IGNITION PARK-SHIFT INTERLOCK FAILURES IN THE SUBJECT VEHICLES, 10 OF WHICH ALLEGE ROLLAWAY CRASHES. IN OCTOBER 2003, AMERICAN HONDA MOTOR COMPANY INITIATED RECALL 03V-423 TO CORRECT AN IGNITION PARK-SHIFT INTERLOCK DEFECT IN CERTAIN MY 1999 HONDA ODYSSEY, MY 1997-99 ACURA CL, MY 1999 ACURA TL, AND MY 1998-99 HONDA ACCORD VEHICLES. IN JANUARY 2005, HONDA INITIATED ANOTHER RECALL TO ADDRESS DEFECTIVE IGNITION PARK-SHIFT INTERLOCKS (RECALL 05V-025), THIS TIME COVERING MY 1997-2001 HONDA PRELUDE, MY 1999-2000 ACURA TL, AND CERTAIN MY 1999-2002 HONDA ACCORD VEHICLES. NOT ALL MY 2002 HONDA ACCORDS WERE COVERED UNDER RECALL 05V-025. THE REMAINING MY 2002 AND ALL MY 2003 ACCORD VEHICLES ARE COVERED IN THE SCOPE OF THIS RECALL QUERY. SOME REPORTS RECEIVED BY ODI AND SOME REPORTS TO GENERAL MOTORS (GM) CONTAINED IN GM'S RESPONSE TO PE08-056 ALLEGE THAT THE ESC SYSTEM MALFUNCTIONED CAUSING A SUDDEN AND INAPPROPRIATE BRAKING ACTION WITHOUT ILLUMINATION OF THE BRAKE LIGHTS AND CAUSING THE VEHICLE TO SWERVE LEFT OR RIGHT UNEXPECTEDLY. GM ISSUED A TECHNICAL SERVICE BULLETIN IN JANUARY 2006, LATER REVISED IN OCTOBER 2007 (BULLETIN NO. 06-02-35-002B) CONCERNING ESC PROBLEMS IN MY 2005 AND 2006 CHEVROLET CORVETTE VEHICLES WITH THE TELESCOPING STEERING WHEEL OPTION. THIS INVESTIGATION PERTAINS TO CERTAIN VOLVO TRUCKS BUILT WITH "SEALED" TRW DRAG LINKS THAT ALLEGEDLY FAIL WITHOUT WARNING, RESULTING IN A STEERING LOSS. THESE DRAG LINKS WERE USED IN THE VN MODEL FROM 1996 THROUGH MY 2005. SEALED LINKS ARE NOT GREASEABLE (NO LUBRICATION REQUIRED) AND THEY ARE STANDARD ON THESE TRUCKS. GREASEABLE LINKS ARE OFFERED AS AN OPTION. PER VOLVO, THE "SEALED" LINKS REQUIRE A QUARTERLY INSPECTION. THAT INSPECTION REQUIRES CHECKING FOR LOOSENESS IN THE STEERING WHEEL AND CHECKING FOR AXIAL MOVEMENT AT THE BALL SOCKETS BY PUSHING AND PULLING THE SOCKET. ANY MOVEMENT REQUIRES REPLACEMENT. THIS INVESTIGATION WAS OPENED AFTER ODI LEARNED ABOUT A FIRE ALLEGED TO HAVE STARTED FROM THE MAIN BATTERY CABLE CHAFING AT THE REAR OF THE ENGINE. THE SUBJECT BUS WAS A NEW 2008 MODEL AND THE BATTERY CABLE RUNS FROM THE ELECTRICAL JUNCTION BLOCK TO THE BATTERY. THE PROBLEM ONLY AFFECTS BUSES EQUIPPED WITH THE CATERPILLAR ENGINE. SUBSEQUENT FIELD INSPECTIONS FOUND BATTERY CABLES ON TWO OTHER BUSES SHOWING SIGNS OF ABRASION AGAINST THE REAR OF THE ENGINE. ON FEBRUARY 16, 2009, BLUE BIRD FILED A SAFETY RECALL TO INSPECT ALL INVOLVED BUSES. BLUE BIRD WILL INSPECT THE BATTERY POWER CABLES AT THE REAR OF THE ENGINE ON THE SUSPECT BUSES AND CORRECT THE PROBLEM AS NECESSARY. THIS RECALL ACTION RESOLVES THE ISSUES RAISED BY THIS INVESTIGATION. ACCORDINGLY, THIS INVESTIGATION IS CLOSED.

ODI'S ROUTINE REVIEW OF EWR FIELD REPORTS REVEALED FAILURES WITH THE HORTON FAN HUB ON CERTAIN PETERBILT TRUCKS. SEPARATION OF THE FAN HUB WOULD RESULT IN A LOSS OF ENGINE COOLING AND POSSIBLY IN COOLING SYSTEM COMPONENTS SEPARATING FROM THE VEHICLE. AT SOME POINT THE VEHICLE WOULD BECOME DISABLED. ADDITIONALLY, THERE IS A POSSIBILITY THAT A LARGE QUANTITY OF ANTIFREEZE WOULD BE SPILLED IN THE VICINITY OF THE DISABLED VEHICLE. THIS ENVIRONMENTAL HAZARD WOULD CREATE A LOW COEFFICIENT FRICTION CONDITION, INCREASING THE HAZARD TO OTHER MOTORISTS. FURTHER, THE POSSIBILITY OF AN INJURY EXISTS WHEN THIS TYPE OF FAILURE TAKES PLACE. MULTIPLE TRUCKING COMPANIES (FLEETS) SUBMITTED THE FIELD REPORTS FURNISHED BY PETERBILT. THE TRUCKING COMPANIES WERE CONTACTED AND IT WAS REVEALED THAT MOST OF THE VEHICLES IN THEIR FLEETS HAD EXPERIENCED THE ALLEGED DEFECT. ODI IS CONCERNED THAT IN THE EVENT OF A FAILURE, A COLLISION WITH A DISABLED SUBJECT VEHICLE AND/OR PERSONAL INJURY MAY RESULT. ODI EXAMINED A SAMPLE SEAT AND FOUND THAT THE BASE RELEASE LEVER MAY NOT OPERATE PROPERLY IN CONJUNCTION WITH THE SEAT TO BASE LATCHING MECHANISM. THE PROBLEM MAY PREVENT PROPER ATTACHMENT OF THE SEAT TO THE BASE. FURTHERMORE, THE OWNER'S MANUAL DOES NOT CONTAIN INFORMATION TO INSTRUCT USERS TO VERIFY THAT THE SEAT IS PROPERLY ATTACHED AFTER CONNECTING THE SEAT TO THE BASE. IF THE SUBJECT SEAT IS NOT PROPERLY ATTACHED TO THE BASE, THE SEAT WILL NOT PROVIDE PROPER PROTECTION FOR THE CHILD DURING A CRASH OR VEHICLE BRAKING AND MANEUVERING.

ON APRIL 14, 2009, HYUNDAI MOTOR CORPORATION SUBMITTED TO ODI A DEFECT INFORMATION REPORT INDICATING THAT IT WOULD RECALL A SALT-BELT POPULATION OF APPROXIMATELY 155,317 MODEL YEAR (MY) 2001 THROUGH 2003 HYUNDAI ELANTRA VEHICLES TO REMEDY A DEFECT THAT CAN RESULT IN LOSS OF VEHICLE CONTROL WITH AN INCREASED RISK OF A CRASH WHILE DRIVING (09V-125). THE AFFECTED VEHICLES COVER ALL MY 2001, MY 2002, AND MY 2003 THROUGH JANUARY 13, 2003 PRODUCTION. THESE INCLUDE ELANTRA VEHICLES THAT WERE ORIGINALLY SOLD OR CURRENTLY REGISTERED IN CONNECTICUT, DELAWARE, DISTRICT OF COLUMBIA, ILLINOIS, INDIANA, IOWA, MAINE, MARYLAND, MASSACHUSETTS, MICHIGAN, MINNESOTA, MISSOURI, NEW HAMPSHIRE, NEW JERSEY, NEW YORK, OHIO, PENNSYLVANIA, RHODE ISLAND, VERMONT, WEST VIRGINIA AND WISCONSIN. THE TOTAL THREE-YEAR PRODUCTION POPULATION FOR THE ELANTRA VEHICLES IS APPROXIMATELY 350,000. ACCORDING TO HYUNDAI, ROAD SALT CAN COLLECT IN THE OUTBOARD PORTION OF THE FRONT LOWER CONTROL ARM AND MAY RESULT IN INTERNAL CORROSION LEADING TO THINNING OF THE STEEL IN THE LOWER CONTROL ARMS. THE CORROSION MAY PROGRESS TO THE POINT WHERE THE LOWER CONTROL ARM UPPER AND LOWER PANELS BECOME PERFORATED. A PERFORATED FRONT LOWER CONTROL ARM MAY FRACTURE BETWEEN ITS BALL JOINT ATTACHMENT AND THE FORWARD AND REARWARD PIVOT ATTACHMENTS TO THE CHASSIS. THIS COULD AFFECT THE DRIVER'S CONTROL OVER THE TIRE AND WHEEL ASSEMBLY. HYUNDAI'S RECALL PROCEDURE INSTRUCTS DEALERS TO MEASURE THE THICKNESS OF THE STEEL IN THE FRONT LOWER CONTROL ARMS. IF SPECIFIED LEVELS OF CORROSION DAMAGE HAVE OCCURRED, THE FRONT LOWER CONTROL ARMS WILL BE REPLACED WITH NEW FRONT LOWER CONTROL ARMS WITH ADDITIONAL HOLES IN THE UPPER AND LOWER PANELS. ODI'S ANALYSIS OF FRONT LOWER CONTROL ARM COMPLAINT AND WARRANTY DATA SHOWS 220 COMPLAINTS AND 1,033 WARRANTY CLAIMS FOR THE TOTAL APPROXIMATELY 350,000 SUBJECT VEHICLES SOLD THROUGHOUT THE UNITED STATES, RESULTING IN A COMPLAINT RATE OF 62.82 PER 100,000 VEHICLES AND A CLAIM RATE OF 0.3 PERCENT. ODI'S ANALYSIS FOUND 7 COMPLAINTS AND 90 WARRANTY CLAIMS IN 132,475 VEHICLES SOLD IN NON-SALT BELT STATES, RESULTING IN A COMPLAINT RATE OF 5.3 PER 100,000 VEHICLES AND A CLAIM RATE OF 0.1 PERCENT. ODI'S ANALYSIS FURTHER FOUND 196 COMPLAINTS AND 943 WARRANTY CLAIMS IN 155,317 VEHICLES SOLD IN SALT BELT STATES, RESULTING IN A COMPLAINT RATE OF 126.2 PER 100,000 VEHICLES AND A CLAIM RATE OF 0.6 PERCENT. ODI IDENTIFIED 34 INCIDENTS OF LOWER CONTROL ARM FRACTURE RELATED TO CORROSION, ALL OF WHICH OCCURRED IN SALT BELT STATES. THE DATA DO NOT SHOW A DEFECT TREND IN VEHICLES OUTSIDE THE REGION COVERED BY HYUNDAI'S RECALL. THIS PRELIMINARY EVALUATION IS CLOSED.

ODI OPENED THIS INVESTIGATION BASED ON THE CONCERN THAT A DISABLED BRAKE IS LIKELY TO CAUSE OR CONTRIBUTE TO AN INCREASE IN THE AFFECTED VEHICLE'S STOPPING DISTANCE WHEN SUBJECTED TO FULL BRAKE APPLICATION, SUCH AS IN AN SITUATION REQUIRING A HIGH SPEED AND/OR EMERGENCY STOP.

ON APRIL 26, 2006 GM NOTIFIED THE AGENCY THAT IT WILL CONDUCT A SAFETY RECALL (NHTSA #06V-139) FOR MODEL YEAR (MY) 2004-2006 COLORADO AND CANYON AND MY 2006 ISUZU I-280 AND I-350 LIGHT DUTY PICKUP VEHICLES. GM INDICATED IN THE PART 573 LETTER THAT THE CAUSE OF THE BRAKE LAMP SWITCH FAILURE WAS INSUFFICIENT PLATING THICKNESS ON THE STATIONARY CONTACT. SINCE THAT TIME, ODI HAS RECEIVED NINE COMPLAINTS FROM SUBJECT VEHICLE OWNERS ALLEGING INTERMITTENT AND REPEATED BRAKE LAMP SWITCH MALFUNCTIONS OF THE RECALL REMEDY SWITCH. SOME OF THE COMPLAINANTS INDICATED CLOSE CALLS FROM APPROACHING VEHICLES AND ONE OWNER (VOQ#10202893) ALLEGED THAT HIS VEHICLE WAS LIGHTLY STRUCK FROM BEHIND AT A STOP LIGHT DUE TO HIS BRAKE LIGHTS NOT WORKING.

SINCE CLOSING THAT INVESTIGATION, ODI HAS RECEIVED ADDITIONAL COMPLAINTS ON MY 2002 IMPREZA WRX AND MY 2003-2004 IMPREZA WRX AND STI VEHICLES. THE MAJORITY OF THESE COMPLAINTS ARE FOR THE MY 2002 TO EARLY-MY 2003 IMPREZA WRX VEHICLES. IN ADDITION, ONE COMPLAINANT SUBMITTED TO ODI A HOME VIDEO DEMONSTRATING LEAKAGE ON A MY 2004 IMPREZA WRX DURING VEHICLE STARTUP IN 5 DEGREE F TEMPERATURE.

THE REAR COMPARTMENT (TRAILER)CONTAINS THE ENGINE AND "PUSHES" THE FRONT COMPARTMENT (TRACTOR). ODI HAS LEARNED THAT A TRANSIT AUTHORITY HAS EXPERIENCED A PARTIAL SEPARATION AT THE ARTICULATING JOINT WELD. THIS JOINT PROVIDES A MEANS OF CONNECTING THE TWO HALVES OF THE BUS TOGETHER AND ALSO PROVIDES A TURNING POINT FOR THE TWO HALVES OF THE BUS. IT IS ALLEGED THAT THE WELD AT THIS POINT FAILED. IF THE JOINT WERE TO COMPLETELY FAIL WHILE THE BUS IS BEING OPERATED, THE TWO SECTIONS OF THE BUS COULD THEN SEPARATE. ODI IS ALSO AWARE OF A SECOND ALLEGATION OF WELD FAILURES ON SIMILAR TYPE BUSES. THIS PE IS OPEN TO FURTHER EVALUATE THESE REPORTED INCIDENTS AND STUDY THE CONSEQUENCES OF THIS TYPE FAILURE.

THIS INVESTIGATION HAS REVEALED THAT FAULTY SIDE AIR BAG MODULE CONNECTORS LOCATED UNDER THE FRONT SEATS CAN CAUSE AN INTERRUPTION IN THE ELECTRICAL CURRENT THAT WILL DROP THE INPUT VOLTAGE BELOW THE SPECIFIED RANGE. AS A RESULT, THE AIR BAG WARNING LAMP ILLUMINATES AND THE AFFECTED SIDE AIR BAG IS DEACTIVATED. THE RATES OF COMPLAINTS, FIELD REPORTS AND WARRANTY CLAIMS RELATED TO THE SUBJECT ISSUE ARE VERY HIGH. FOR EXAMPLE, NISSAN SUBMITTED 4,853 WARRANTY CLAIMS ON 3,551 DISTINCT SUBJECT VEHICLES OUT OF 13,836 TOTAL SUBJECT VEHICLES. ALSO, ANALYSES OF WARRANTY CLAIMS INDICATE AN INCREASING FAILURE TREND OVER TIME. AN ENGINEERING ANALYSIS HAS BEEN OPENED TO FURTHER STUDY THIS ISSUE.

(HALDEX) REPORTED THAT CERTAIN AIR BRAKE SYSTEM CONTROL VALVES, DISTRIBUTED IN 2008, CONTAIN A RUBBER DIAPHRAGM INSIDE THE QUICK RELEASE VALVE THAT MAY DELAMINATE. DELAMINATION OF THE DIAPHRAGM MAY CAUSE SIGNIFICANT LEVELS OF LEAKAGE AND/OR LEAD TO THE DEVELOPMENT OF A BUBBLE IN THE DIAPHRAGM THAT COULD TRAP AIR PRESSURE IN THE VALVE. THIS CONDITION CAN PREVENT A VEHICLE'S SERVICE BRAKES FROM RELEASING PROPERLY. MALFUNCTIONING SERVICE BRAKES CAN LEAD TO RISK OF A VEHICLE CRASH. NHTSA ASSIGNED RECALL NUMBER 09E-004 TO THIS MATTER. IN THEIR DEFECT REPORT, HALDEX NOTED THAT 20 VEHICLE MANUFACTURERS PURCHASED THIS EQUIPMENT AS WELL AS 10 DISTRIBUTORS. THIS INVESTIGATION IS BEING OPENED TO CONTACT EACH OF HALDEX'S CUSTOMERS, NOTIFY THEM OF THIS RISK TO MOTOR VEHICLE SAFETY, AND REMIND THEM OF THEIR RESPONSIBILITIES TO NOTIFY NHTSA AND THEIR CUSTOMERS OF ANY VEHICLES THEY MANUFACTURED CONTAINING THIS EQUIPMENT.

ODI HAS RECEIVED 64 CONSUMER REPORTS AND TOYOTA HAS RECEIVED 96 REPORTS WHICH ALLEGE INCIDENTS OF INAPPROPRIATE ACTIVATION OF EITHER THE ELECTRONIC STABILITY CONTROL SYSTEM [TOYOTA NOMENCLATURE: VEHICLE STABILITY CONTROL (VSC)] AND/OR THE TRACTION CONTROL SYSTEM IN MODEL YEAR (MY) 2003 TOYOTA SEQUOIA VEHICLES. DURING THE ACTIVATION OF EITHER SYSTEM THE DRIVER LOSES THROTTLE CONTROL AND ONE OR MORE BRAKES MAY APPLY CAUSING THE VEHICLE TO SLOW SUDDENLY. DURING THESE ACTIVATION EVENTS THE BRAKE LIGHTS ARE NOT ILLUMINATED TO SIGNAL TO FOLLOWING TRAFFIC THAT THE VEHICLE IS SLOWING. SOME OF THE COMPLAINANTS ALLEGE ALMOST BEING STRUCK BY FOLLOWING OR CROSSING TRAFFIC BECAUSE OF THE UNEXPECTED DECELERATION. IN RESPONSE TO ODI'S INFORMATION REQUEST LETTER FOR PE08-072, TOYOTA INDICATED THAT IT HAS REVIEWED THE REPORTS SUBMITTED TO ODI AND TO TOYOTA AND THAT IT BELIEVES THAT THERE ARE SEVERAL ISSUES RESPONSIBLE FOR THE REPORTED INCIDENTS. THE MANUFACTURERS' REMEDY IS TO INSPECT THE ABS CONTROL MODULE ELECTRICAL CONNECTOR TO DETERMINE IF ANY PIN SEALS ARE MISSING OR DISLODGED OR IF THERE IS CORROSION ON THE PINS. IF THERE IS PIN CORROSION THE ABS CONTROL MODULE IS REPLACED (PENDING PART AVAILABILITY), A NEW PIGTAIL CONNECTOR IS SPLICED INTO THE WIRING HARNESS AND ELECTRICAL GREASE IS APPLIED TO THE ABS CONTROL MODULE CONNECTOR. IF PIN SEALS ARE MISSING OR IF THE CONNECTOR HAS THE WRONG MAT SEAL NUMBER, A NEW PIGTAIL CONNECTOR IS SPLICED INTO THE WIRING HARNESS AND ELECTRICAL GREASE IS APPLIED TO THE ABS CONTROL MODULE CONNECTOR. IF ANY PIN SEALS ARE OUT OF POSITION, THE CONNECTOR WILL BE REPAIRED BY SEATING THE DISLODGED SEALS AND ELECTRICAL GREASE IS APPLIED TO THE ABS CONTROL MODULE CONNECTOR. IF THE INSPECTION REVEALS NO PIN CORROSION, NO MISSING OR DISLODGED PIN SEALS, AND THE CORRECT MAT SEAL NUMBER THEN THE REMEDY CONSISTS SOLELY OF APPLYING ELECTRICAL GREASE TO THE ABS CONTROL MODULE CONNECTOR. THE OFFICE OF DEFECTS INVESTIGATION (ODI) RECEIVED A DEFECT PETITION DATED MARCH 5, 2009, ALLEGING THAT THE PETITIONER NEVER RECEIVED THE RECALL REMEDY FOR 07V-156 AND THAT, AS A RESULT, THE PETITIONER'S MY2003 FORD ESCAPE CAUGHT FIRE AT THE ABS CONTROL MODULE. THE MANUFACTURER VERIFIED THAT THE PETITIONER'S VEHICLE HAD INDEED BEEN SERVICED UNDER THE RECALL CAMPAIGN. TO DATE, THE AGENCY IS AWARE OF AT LEAST 6 NON-CRASH FIRE INCIDENTS IN THE SUBJECT VEHICLES ALLEGED TO HAVE ORIGINATED IN THE ENGINE COMPARTMENT AT OR NEAR THE ABS CONTROL MODULE DESPITE THE RECALL REMEDY HAVING BEEN APPLIED. ACCORDINGLY, A RECALL QUERY HAS BEEN OPENED TO ASSESS THE EFFECTIVENESS OF THE RECALL REMEDY.

NAVISTAR STATED VEHICLES WHOSE USE WAS OCCASIONAL OR SEASONAL, SUCH AS AN RV, AND WHICH WERE EQUIPPED WITH THESE BRAKES MAY EXPERIENCE CALIPERS STICKING IN THE APPLIED POSITION. THIS CAN RESULT IN BRAKE DRAG, ABNORMAL HEAT GENERATION, BRAKE FLUID BOIL, AND EXTENDED STOPPING DISTANCES, INCREASING THE RISK OF A CRASH. NHTSA ASSIGNED RECALL NUMBER 09V-110 TO THIS RECALL. IT IS UNKNOWN HOW MANY OTHER VEHICLE MANUFACTURERS HAVE RECEIVED THESE HYDRAULIC DISC BRAKES PRODUCED BY BOSCH. THIS INVESTIGATION IS BEING OPENED TO OBTAIN A LIST OF OTHER VEHICLE MANUFACTURERS THAT WERE SOLD THIS EQUIPMENT, WRITE TO THOSE MANUFACTURERS REQUESTING SUBSEQUENT DEFECT REPORTS FOR VEHICLES WHEN APPLICABLE, AND TO REQUEST BOSCH TO SUBMIT THE PROPER 573 DEFECT INFORMATION REPORT PER 49 CFR §573.3 (F) SHOULD THEY AGREE THIS EQUIPMENT IS DEFECTIVE.
