Summary:
IN RESPONSE TO AN INFORMATION REQUEST LETTER SENT BY THE OFFICE OF DEFECTS INVESTIGATION (ODI) DURING PRELIMINARY EVALUATION PE08-010, GM PROVIDED ODI WITH 41 ALLEGATIONS OF NON-CRASH ENGINE COMPARTMENT FIRES INCLUDING 16 ALLEGATIONS OF FIRES ORIGINATING WITH THE IGNITION IN THE OFF POSITION. ONE OF THE FIRES WAS DETERMINED TO BE CAUSED BY AN ARSON EVENT. ODI HAS ALSO RECEIVED 5 REPORTS, 4 OF WHICH ARE OF DUPLICATIVE OF REPORTS PROVIDED BY GM, ALLEGING NON-CRASH ENGINE COMPARTMENT FIRES IN THE SUBJECT VEHICLES. IN TOTAL, ODI IS AWARE OF 41 NON-CRASH ENGINE COMPARTMENT FIRES IN THE SUBJECT VEHICLES THAT ARE UNRELATED TO ARSON. EIGHT OF THE 41 FIRE ALLEGATIONS ALSO ALLEGE PROPERTY DAMAGE AS A RESULT OF THE FIRES. PRELIMINARY EVALUATION PE08-010 HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS TO FURTHER ASSESS THE SCOPE, FREQUENCY, ORIGIN AND CAUSE OF THE ALLEGED NON-CRASH RELATED ENGINE COMPARTMENT FIRES IN THE SUBJECT VEHICLES.

IN A NOVEMBER 7, 2007 LETTER, GENERAL MOTORS SUBMITTED A DEFECT INFORMATION REPORT TO ODI CONCERNING THE RECALL OF APPROXIMATELY 20,500 MY 2001 SATURN L-SERIES VEHICLES WITH 2.2L ENGINES THAT WERE BUILT BETWEEN NOV. 2000 AND FEB. 2001 TO CORRECT A PROBLEM WITH TIMING CHAIN LINK SEPARATION (RECALL 07V-519, ODI INVESTIGATION EA06-009). TIMING CHAIN FAILURE COULD RESULT IN ENGINE STALL WHILE DRIVING WITH NO CHANCE FOR A RESTART. THE TIMING CHAIN ASSEMBLY USED IN THE RECALLED VEHICLES WAS ALSO USED IN APPROXIMATELY 391,600 OTHER MY 1999 THROUGH 2003 SATURN L-SERIES VEHICLES WITH 2.2L ENGINES. HOWEVER, WHILE THE RECALLED VEHICLES REPRESENTED ONLY ABOUT 5% OF THE TOTAL POPULATION, THEY EXPERIENCED AN ELEVATED "SPIKE" IN TIMING CHAIN FAILURES ACCOUNTING FOR ABOUT 32% OF THE TOTAL COMPLAINTS TO ODI AND GM (384 OF 1200) AND 25% OF TOTAL TIMING CHAIN WARRANTY CLAIMS (971 OF 3,903) WHEN EA06-009 WAS CLOSED ON NOVEMBER 9, 2007. THE RESULTING INCIDENT RATES WHEN EA06-009 WAS CLOSED WERE 1.9% VS 0.2% FOR COMPLAINTS AND 4.7% VS 0.7% FOR WARRANTY CLAIMS, RESPECTIVELY, FOR THE RECALLED AND NON-RECALLED POPULATIONS. ON MAY 22, 2008, THE NORTH CAROLINA CONSUMERS COUNCIL (NCCC) SUBMITTED A DEFECT PETITION TO ODI REQUESTING THE EXPANSION OF RECALL 07V-519 TO INCLUDE ALL MY 1999 THROUGH 2003 SATURN L-SERIES VEHICLES WITH 2.2L ENGINES USING THE SAME TIMING CHAIN DESIGN AS THE RECALLED POPULATION. THE PETITION STATES THAT NCCC CONTINUES TO RECEIVE COMPLAINTS FROM CONSUMERS THAT HAVE HAD A TIMING CHAIN FAIL IN VEHICLES BUILT OUTSIDE THE PRODUCTION RANGE COVERED BY THE RECALL. THE PETITION ALSO NOTES THAT ODI HAS CONTINUED TO RECEIVE COMPLAINTS. SINCE THE CLOSING OF EA06-009 ODI HAS RECEIVED 51 NEW COMPLAINTS FROM CONSUMERS WHO HAD A TIMING CHAIN FAIL IN A MY 1999 THROUGH 2003 SATURN L-SERIES WITH THE 2.2L ENGINE THAT WERE NOT COVERED BY RECALL 07V-519, BRINGING THE CURRENT TOTAL TO 216. THE PETITIONER NOTES THAT THE VEHICLES THAT FALL OUTSIDE THE RECALLED GROUP HAVE THE SAME TIMING CHAIN AS THE VEHICLES THAT WERE RECALLED AND EXHIBIT THE SAME FAILURE MODE WHERE THE TIMING CHAIN CAN BREAK SUDDENLY AND WITHOUT WARNING, RESULTING IN ENGINE DAMAGE AND LOSS OF MOTIVE POWER. BECAUSE OF THIS, THE PETITIONER BELIEVES THAT THE ORIGINAL SCOPE OF THE RECALL SHOULD BE CHANGED TO INCLUDE ALL VEHICLES BUILT WITH THE ORIGINAL TIMING CHAIN PART. IN RECALL 04V442, FORD REPLACED STRUT MOUNTING BRACKETS AND HINGES ON MODEL YEARS 2002-2003 EXPLORERS BECAUSE THE REAR LIFTGATE GLASS STRUT COULD DISENGAGE OR THE GLASS HINGE COULD FRACTURE. TIRE INFLATION PRESSURE IS CRITICALLY DEPENDENT ON OWNERS RECEIVING AND UNDERSTANDING THE IMPORTANCE OF THIS STRINGENT AND ONGOING MAINTENANCE REQUIREMENT. SPECIFICALLY THEY STATED THAT THEIR DECISION WAS BASED ON ONE WARRANTY CLAIM, WHICH PACE LATER DETERMINED TO BE THE RESULT OF OVERLOADING AND MISINFORMATION FROM TREDIT, WHICH ALLUDED TO SIMILAR WARRANTY PROBLEMS WITH OTHER CUSTOMERS. PACE STATED THAT IT UNDERSTANDS TREDIT'S OTHER CUSTOMER WARRANTY CLAIMS TO BE A RESULT OF OVERLOADING THE KITCHEN AREA IN TRAVEL TRAILERS MANUFACTURED BY WESTERN RV AND DOUBLE TREE RV SUITES. PACE INFORMED NHTSA THAT THE WHEELS INVOLVED IN THE RECALL WERE MANUFACTURED IN ARGENTINA BY RUEDAS EV SRL, IMPORTED BY U.S. WHEEL, SOLD TO TREDIT, AND SUPPLIED TO PACE. THE PURPOSE OF THIS INVESTIGATION IS TO REQUEST MORE INFORMATION RELATING TO THE WHEELS IN QUESTION AND PACE'S RECALL DECISIONS.

AS A RESULT, "THE SWITCH MAY START TO HEAT UP AND MELT OR IGNITE THE PLASTIC COMPONENTS." HOWEVER, MITSUBISHI ALSO ASSERTS THAT THE ALLEGED DEFECT IS "VERY RARE" AND THAT "BURNING DAMAGE WILL NOT OCCUR UNLESS LIQUIDS ENTER INTO THE" POWER WINDOW SWITCH. ODI OPENED THIS ENGINEERING ANALYSIS TO FURTHER EVALUATE THE ISSUE.

UNDER INVESTIGATION PE08-017, HYUNDAI PROVIDED 472 OWNER REPORTS AND 9,108 WARRANTY CLAIMS RELATED TO THESE ISSUES FOR MY 2001 AND 2002 VEHICLES BUILT THROUGH 12/14/2001. ODI IS ALSO AWARE OF SIX INCIDENTS OF SEAT BELT PRETENSIONER/AIR BAG INADVERTENT DEPLOYMENTS DUE TO LIQUID SPILLS. TO ADDRESS THE ABOVE ISSUES, ON 04/25/2001, HYUNDAI ADDED A CARPET COVER/SHIELD OVER THE AIR BAG CONTROL MODULE CONNECTOR AREA AND UPGRADED THE CABLE ROUTING AND ATTACHMENT POINTS TO IMPROVE THE ELECTRICAL CONNECTIONS ON VEHICLES BUILT AFTER 12/14/2008. MANY OF THE COMPLAINTS SPECIFY THAT THE SECONDARY HOOD LATCH COMPONENTS WERE CORRODED. THE CORROSION APPEARS TO PREVENT THE LATCH FROM OPERATING PROPERLY. IF THE SECONDARY HOOD LATCH BINDS WHEN THE HOOD IS OPENED, IT MAY NOT ENGAGE THE LOCKING COMPONENTS WHEN THE HOOD IS THEN CLOSED. IN MOST CASES, SECONDARY HOOD LATCH MALFUNCTION WOULD NOT BE OBVIOUS TO THE DRIVER. IF THE PRIMARY HOOD LATCH WERE TO FAIL OR NOT BE ENGAGED, THE HOOD COULD OPEN UNEXPECTEDLY WHILE THE VEHICLE IS UNDER WAY. SUDDEN AND UNEXPECTED OPENING OF THE HOOD STARTLES DRIVERS, BLOCKS THEIR VISIBILITY, AND COULD LEAD TO A CRASH INTO OTHER VEHICLES OR OBJECTS.

SHOULD THIS SYSTEM NOT OPERATE PROPERLY, THE WHEELCHAIR LIFT OCCUPANT MIGHT FALL FROM THE LIFT AND SUFFER INJURY. RICON SUBMITTED ANOTHER NONCOMPLIANCE REPORT (07E-097) FOR THE S SERIES & K SERIES WHEELCHAIR LIFTS, ENCOMPASSING SOME OF THE LIFTS FROM THE ABOVE RECALL. THE OFFICE OF DEFECTS INVESTIGATION HAS RECEIVED ONE FLEET COMPLAINT IDENTIFYING 33 ALLEGED FAILURES ON THE SUBJECT WHEELCHAIR LIFTS. THE ALLEGED FAILURES HAVE OCCURRED ON SCHOOL BUSES IN TWO ALASKAN SCHOOL DISTRICTS. THESE ALLEGATIONS DESCRIBE CRACKS DEVELOPING ON THE LIFTS' VERTICAL SUPPORT ARMS AT A PINNED JOINT WHERE A HORIZONTAL ARM INTERFACES WITH THE VERTICAL ARM. THE LIFTS ARE USED TO RAISE WHEELCHAIRS FROM THE STREET LEVEL TO ALLOW ENTRY INTO THE SCHOOL BUS AND ALSO TO LOWER WHEELCHAIRS FROM THE SCHOOL BUS BACK TO GROUND LEVEL. THESE BENT ARMS MAY CAUSE THREE POSSIBLE SAFETY CONCERNS. FIRST, THE BENT ARMS MAY PREVENT AN INBOARD BRIDGE FROM LAYING FLAT AND THUS PREVENT A WHEEL CHAIR FROM LOADING THE LIFT IN AN EMERGENCY. SECOND, BENT ARMS MAY CAUSE THE ANTI-STOW INTERLOCK SWITCH TO BECOME OUT OF ADJUSTMENT POSSIBLY ALLOWING THE LIFT TO START MOVING TO THE STOWED POSITION WHILE A CHAIR IS STILL ON THE PLATFORM. THIRD, THE BENT ARMS MAY ALSO CREATE A SITUATION WHERE THE REQUIREMENTS OF FMVSS 403 - S6.10.2.3 ARE NO LONGER MET. RICON FEELS THAT THE BENT ARMS ARE NOT A SAFETY CONCERN BECAUSE THE OPERATOR WILL SEE THAT THE LIFT IS DAMAGED BEFORE A PASSENGER IS LOADED ONTO THE BUS. IF THE DRIVER SEES THAT THE LIFT IS DAMAGED, THEY SHOULD NOT LOAD A PASSENGER WHO WOULD THEN NEED TO BE UNLOADED IN AN EMERGENCY. THIS PE HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS (EA) TO DETERMINE WHAT CAUSES THE ARMS TO FAIL, IF THE ARMS CAN FAIL WHILE THE LIFT IS IN A STOWED POSITION, AND IF A PASSENGER CAN BE LOADED WHILE THE LIFT IS IN A FAILED CONDITION.

Summary:
IN A JUNE 25, 2008, LETTER RESPONDING TO AN INFORMATION REQUEST FROM ODI, TOYOTA REPORTED AN APRIL 2003 UNWANTED ACCELERATION INCIDENT THAT OCCURRED DURING PRODUCTION DYNAMOMETER TESTING. ACCORDING TO THE LETTER, THE INCIDENT WAS CAUSED BY A MISSING RETAINING CLIP THAT ALLOWED THE CENTER CONSOLE TRIM PANEL TO INTERFERE WITH (TRAP) THE ACCELERATOR PEDAL AFTER IT HAD BEEN DEPRESSED. IN THE AFTERMATH, TOYOTA REVIEWED THEIR MANUFACTURING PROCESSES AND OTHER DATA AND CONCLUDED THIS WAS AN ISOLATED INCIDENT. AS A SECONDARY MEASURE IN JUNE 2003, TOYOTA CHANGED THE DESIGN OF THE TRIM PANEL TO ELIMINATE THE POTENTIAL FOR PEDAL INTERFERENCE IN THE EVENT THE RETAINING CLIP IS NOT PRESENT. DURING A JULY 2008 MEETING WITH ODI, TOYOTA DEMONSTRATED HOW AN UNRESTRAINED EARLY DESIGN LEVEL TRIM PANEL INTERACTS WITH THE ACCELERATOR AFTER PEDAL DEPRESSION. TOYOTA ALSO ADVISED THAT THEY WERE CONDUCTING A FIELD SURVEY TO EXAMINE PANEL RETENTION AND THAT PRELIMINARILY ONE VEHICLE HAD BEEN IDENTIFIED WITH A CONCERN. NHTSA'S VEHICLE RESEARCH AND TEST CENTER IS ALSO CONDUCTING A SURVEY OF SUBJECT VEHICLES; PRELIMINARILY TWO RESPONSES ALLEGE A CONCERN. BEFORE OPENING PE08-025, ODI STAFF ALSO FOUND A SUBJECT VEHICLE WITH A MISSING RETAINING CLIP. NOTE THAT ODI IS NO LONGER COUNTING THE SINGLE COMPLAINT ALLEGING TRIM PANEL INTERFERENCE THAT WAS IN THE OPENING RESUME FOR PE08-025, AS IT WAS DETERMINED THAT THE VEHICLE INVOLVED WAS NOT EQUIPPED WITH THE SUBJECT TRIM PANEL. PE08-025 HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS TO GATHER ADDITIONAL INFORMATION ABOUT THE NUMBER OF SUBJECT VEHICLES THAT MAY CURRENTLY HAVE ISSUES WITH THE RETAINING CLIP AND TO ASSESS THE FUTURE RISK OF PEDAL INTERFERENCE INCIDENTS DUE TO MISSING CLIPS.

Summary:
THE LIFTGATE GAS STRUTS FOR THE SUBJECT VEHICLES MAY HAVE DAMAGED GAS SEALS THAT ALLOW THE LIFTGATE TO CLOSE UNEXPECTEDLY. AT THE COMPLETION OF A POWER-OPEN COMMAND, THE POWER LIFTGATE MAY EITHER CLOSE WITHOUT POWER ACTUATION (I.E., DESCEND UNASSISTED UNDER THE FORCE OF GRAVITY) OR AUTOMATICALLY POWER-CLOSE WITHOUT INPUT FROM THE OPERATOR. SOME OWNERS ARE CAUGHT UNAWARE OF THE UNINTENDED CLOSING OF THE LIFTGATE WHILE UNDER THE REAR LIFTGATE DOOR. THERE HAVE BEEN TEN REPORTED INJURIES RELATED TO THE ALLEGED PROBLEM ON THE SUBJECT VEHICLES.

ODI HAS RECEIVED THREE COMPLAINTS FROM OWNERS OF PEG PEREGO MODEL PRIMO VIAGGIO INFANT CAR SEATS IN THE LAST TWO MONTHS. IN ALL THREE COMPLAINTS, INFANTS CUT THEIR FEET BY RUBBING THEM ON AN EDGE OF MOLDING FLASHING ON THE PLASTIC HARNESS ADJUSTER HOUSING. THE EDGE ON THE HARNESS ADJUSTER HOUSING IS LOCATED IN AN AREA ACCESSIBLE TO THE FEET OF AN INFANT IN THE SEAT. IN ALL THREE COMPLAINTS, THE INFANTS SUSTAINED LACERATIONS TO THE BACK OF THEIR FEET AT THE HEEL OR ANKLE. THE PEG PEREGO PRIMO VIAGGIO IS A REAR-FACING INFANT SEAT THAT HAS A CARRIER THAT ATTACHES TO A BASE. THE SEAT CAN BE USED FOR INFANTS UP TO 30 POUNDS.

THIS ISSUE IS ALSO THE SUBJECT OF NUMEROUS WEB FORUM POSTS. OUR DISCUSSIONS WITH DEALERS INDICATE OTHER SUBJECT VEHICLE OWNERS HAVE EXPERIENCED THIS PROBLEM TOO.
