Eva Foam Puzzle Mats with Numbers or Letters
Full Product Description


This recall involves three puzzle sets of coloured foam mats, where each puzzle piece has a removable foam cut-out in the shape of either a number or an alphabet letter.

The puzzle set is sold in a clear plastic bag with the product name "Eva Puzzle Mats" printed on the packaging. The UPC and item number can also be found on a sticker on the bag.

One puzzle set has 10 square foam puzzle pieces with cu-touts in the shape of numbers. This set can be identified by UPC 773392098367 and item number TO-2371.
One puzzle set has 36 rectangular foam puzzle pieces with cut-outs in the shape of alphabet letters and numbers. This set can be identified by UPC 773392098343 and Item number TO-2369.
One puzzle set has 36 small foam puzzle pieces with cut-outs in the shape of alphabet letters and numbers. This set can be identified by UPC 773392098350 and Item number TO-2370.

Hazard Identified

Health Canada's sampling and evaluation program has revealed that the puzzle sets can break into small parts, posing a choking hazard to young children under 3 years of age.

Health Canada has not received any reports of incidents or injuries related to the use of this product.

Corrective Action

Consumers should immediately take away the recalled toy from children and dispose of it in regular household garbage.
Number Sold

Approximately 22,171 sets of the puzzle with numbers and alphabet letters were sold at dollar stores in Quebec.
Time Period Sold

The recalled products were sold from November 2006 to December 2007.
